Who should complete and sign the Claim Form?

The on-line Claim Form must be completed by the beneficial owner of Facebook Common Stock (or their authorized representative) with respect to which the Claim Form is being filed. If there is more than one beneficial owner of the Facebook Common Stock, all beneficial owners must be identified and all owners (or an authorized legal representative of such owners) must sign the Claim Form.

What will you need to proceed?

• The beneficial owner’s name(s), mailing address and telephone number.
• A complete list of purchases and sales of Facebook Common Stock from January 28, 2016 through June 15, 2018 , as well as the number of shares held on January 27, 2016 and on June 15, 2018.
• Supporting documentation for each transaction. All documentation must be legible and include your name, account number, date of transaction, quantity, total transaction amount and security name or CUSIP. You may choose to:
  • Attach scanned image files (up to 10 separate files), or
  • Complete your online submission without attaching documentation and mail us paper copies (via post or courier).
• If the Claim Form is being submitted by the beneficial owner’s authorized representative, you must provide proof of your authority to act on their behalf.

The online Claim Form is meant to be completed within a single 60-minute session. It is advised that all necessary information and scanned documentation be readily accessible before you begin your claim.
